Additional Support Services
Entitlement Specialists
CCMHC employs a full-time Entitlement Specialists to assist you with applying for benefits for which you may be eligible. They can assist you with completing applications, collecting documentation, and with following up with claims when needed. Every effort is made to ensure all patients obtain benefits for which they are entitled.
Care Coordination
Care coordinators are available to assist you in connecting with other service providers in the community. The Care Coordinator will meet with you to determine the needs you have and will work with you to plan the services you need and where you can get them. Based on your plan, the Care Coordinator will make referrals and follow up with you and other providers to make sure your needs are being met. Medical care, dental care, food, clothing, and educational services are some examples of how Care Coordination services can help you.
Individual Placement & Support
Individual Placement and Support (IPS) is an evidence based practice for employment in collaboration with Vocational Rehab. We believe that most people with mental illness want to work and considers this an important part of recovery. Benefits are many; including increased income, improved self-esteem, improved social networks, increased quality of life, better control of symptoms, reduced hospitalizations, etc. Your Employment Specialist is with you every step of the way through the process of obtaining and sustaining competitive employment of your choice.
Peer Support
Peer Support Specialists are available to assist individuals age 18 and over in understanding how to manage their illness in their daily lives to promote ongoing recovery. Services focus on personal safety, self-worth, choice, confidence, growth, education, meaningful activities and effective communication skills.
Primary Care Services
Catawba CMHC now partners with North Central Medical to offer primary care services at our clinics. This embedded FQHC (Federally Qualified Health Center) offers treatment of acute and chronic disease management, preventative care and education provided by a licensed medical professional.
